Simply call the Membership Coordinator, Ann Kennedy at 416-422-5644 ext. 249 (Toll Free 1-877-422-1112) to complete the form over the phone. 

By Mail
Download and complete the attached membership form and mail it with a cheque payable to CPA Ontario / credit card information.
